A new approach to managing Chronic Conditions

This document describes a new model of community centred care for people living with chronic conditions. The driving vision behind this model is to deliver the appropriate level of care, administered by the appropriate person or system, in the most appropriate setting. This model of care is based on a new process of referral that is based on risk scores, predictive modelling and patient choice, where a patient segmentation is performed the outcome of which leads to a range of personal interventions and services.
